Public Service form three PSF3, (Revised 2008) should be filled in triplicate with 3 recent colored Passport Photos, Certified Copies of Academic Documents, Registration Certificates, Practicing Licenses, Academic and National IDs MUST be attached and submitted to the Secretary, Lyantonde District Service Commission, P. O. Box 10, Lyantonde. Not later than 24th April 2026 stating the post applied for and its reference number.
